Under IRS rules, The LeRoy Ostrander Public School is providing notice regarding your 2025 Form 1095-C, which contains information about the health insurance coverage offered to you.
You are not required to have this form to file your tax return, but you may request a copy for your records.
How to Request Your Form 1095-C
If you would like a copy of your 2025 Form 1095-C, you may request it by:
Please include your full name and employee ID, if applicable. Forms requested will be mailed to your address on file or provided electronically within 30 days of the request.
This notice will remain posted on our website at www.leroy.k12.mn.us until October 15, 2026.
